Life Event: Gain or Loss of Eligibility for TRICARE

This Life Event is when any of the below scenarios take place; expand the relevant menu for guidance.

When you become eligible for and enroll in TRICARE, you can waive your enrollment in medical coverage.

To do so, complete these three steps no later than 60 days after gaining TRICARE eligibility or the date the relevant WA State agency (e.g. HCA, PEBB, etc.) grants TRICARE eligibility:

  1. Download and complete the Employee Enrollment Change form linked in the right-hand sidebar of this page.
  2. Obtain a Certificate of Creditable Coverage or proof of enrollment from TRICARE.
  3. Send the completed form and supporting documentation to UWHR Benefits either via email (preferred) or mail.

Effective Dates
The effective date of the change is the last day of the month of the later of either:

  1. The date you gained eligibility for and enrolled in TRICARE
    -OR-
  2. The date the Employee Enrollment Change form is received by UWHR Benefits

If the later of a. or b. is the 1st day of the month, the effective date is the last day of the previous month.

When your spouse/SRDP becomes eligible for, and enrolls you as a dependent in, their TRICARE coverage,* you can waive your enrollment in medical coverage.

*Dependent children who gained eligibility for TRICARE may not be removed from your medical and/or dental coverage.

To do so, complete these three steps no later than 60 days after gaining TRICARE eligibility or the date the relevant WA State agency (e.g. HCA, PEBB, etc.) grants TRICARE eligibility:

When you lose your eligibility for TRICARE, if you had previously waived medical coverage, you can enroll in medical coverage (i.e. “return from waived enrollment”).

To do so, complete these three steps no later than 60 days after losing your TRICARE eligibility:

  1. Download and complete the Employee Enrollment Change form linked in the right-hand sidebar of this page.
  2. Obtain a Certificate of Creditable Coverage or proof of termination of coverage from TRICARE.
  3. Send the completed form and supporting documentation to UWHR Benefits either via email (preferred) or mail.

Effective Dates
The effective date of the change is the 1st of the month following the later of either:

  1. The date you lost your eligibility for TRICARE
    -OR-
  2. The date the Employee Enrollment Change form is received by UWHR Benefits

If the later of a. or b. is the 1st day of the month, the change takes effect on that day.

When your dependent loses their eligibility for TRICARE, if you had previously waived medical coverage, you can enroll in medical coverage (i.e. “return from waived enrollment”) in order to add the dependent who lost their TRICARE eligibility to your medical coverage.

To do so, complete these three steps no later than 60 days after your dependent lost their TRICARE eligibility:

  1. Download and complete the Employee Enrollment Change form linked in the right-hand sidebar of this page.
  2. Obtain a Certificate of Creditable Coverage or proof of termination of coverage from TRICARE.
  3. Send the completed form and supporting documentation to UWHR Benefits either via email (preferred) or mail.

Effective Dates
The effective date of the change is the 1st of the month following the later of either:

  1. The date your dependent lost their eligibility for TRICARE
    -OR-
  2. The date the Employee Enrollment Change form is received by UWHR Benefits

If the later of a. or b. is the 1st day of the month, the change takes effect on that day.
