On May 5, 2020 the University announced a new Working On-Site COVID-19 Symptom Attestation as part of the ongoing efforts to reduce the spread of COVID-19. Completing the attestation is required for all UW employees, contingent workers, stipend recipients and others who work on-site at the UW but are not employees, confirming that they do not have any COVID-19-related symptoms. The attestation does not need to be completed by employees who are teleworking.
The attestation is now available in Workday. To start the attestation, employees can log into Workday and select the “Working On-Site Attestation” announcement on the Workday homepage. Medical Centers employees do not need to take the attestation in Workday, but rather should follow their own Medical Center attestation guidance.
In addition to the attestation, a new COVID-19 Attestation Report has also been developed for Supervisors. Supervisors can log into Workday and select the “COVID-19 Attestation” worklet to review which of their employees working on-site have completed the attestation. Alternatively, Supervisors can run the report by entering “R0654 Employee COVID-19 Attestation Daily Audit for Managers” in the search bar in Workday. Supervisors who wish to see a list of all individuals in their Supervisory Organization (Sup Org) can use the My Team worklet found on their Workday homepage or run the R0115 General Employee Roster report.
