Med Centers employees: Follow UW Medicine procedures for verifying your vaccination status instead of following the steps on this page.
Student employees: Verify your vaccination status using the Student COVID-19 Vaccination Form in addition to following the steps on this page.
On June 3, 2021, the University announced that all personnel must be vaccinated against COVID-19 by the autumn quarter in order to work on-site at any of the campuses or within UW facilities. If you are fully vaccinated, or if you are declaring an exemption for medical reasons or for religious or philosophical objections, you can confirm your vaccination or exemption status in Workday using the steps described below.
Learn more about vaccinations and the vaccine requirement on the UW’s COVID-19 Vaccinations page.
Not fully vaccinated yet?
Please wait until you are fully vaccinated before completing your vaccine attestation in Workday. You are considered fully vaccinated two weeks after receiving the second dose in a two-dose vaccine series such as Pfizer or Moderna, or two weeks after a single-dose vaccine such as Johnson & Johnson.
