In order to protect the health and safety of our community, the University of Washington is requiring all students and employees to be vaccinated against COVID-19, with certain exemptions allowed. You can now complete your vaccine verification form in Workday.

Completing Your COVID-19 Vaccination Attestation in Workday

On June 3, 2021, the University announced that all personnel must be vaccinated against COVID-19 by the autumn quarter in order to work on-site at any of the campuses or within UW facilities. If you are fully vaccinated, or if you are declaring an exemption for medical reasons or for religious or philosophical objections, you can confirm your vaccination or exemption status in Workday using the steps described below.

  • Not fully vaccinated yet?
    Please wait until you are fully vaccinated before completing your vaccine attestation in Workday. You are considered fully vaccinated two weeks after receiving the second dose in a two-dose vaccine series such as Pfizer or Moderna, or two weeks after a single-dose vaccine such as Johnson & Johnson.


Submit Your Attestation Response

1. Log into Workday using the Sign In To Workday link in the upper right corner of this page.

2. On your Workday homepage, select the COVID-19 Vaccination Attestation link.

3. On the next screen, locate the COVID-19 Vaccine Verification section, and select the Edit button (select the image below for a larger version).

4. Specify your attestation response per the guidance in the appropriate menu below.

Make all of the following selections:

A. Check the I am fully vaccinated box. (You can ignore the error messages that appear on the page; they will disappear as you complete the remainder of form.)
B. In the Vaccine Manufacturer field, select the appropriate choice.
If you had two different types of vaccination shots (for example, your first shot was Moderna and your second was Pfizer), select the manufacturer of your final shot.
C. In the Final Vaccination Date field, select the date on which you received your final shot.
D. Check both the I declare… and I acknowledge… boxes.

If you are declaring an exemption from the COVID-19 vaccination requirement for medical reasons or for religious or philosophical objections, make all of the following selections. Please note that unvaccinated personnel, even with a registered exemption, must wear a mask when working on-site at UW facilities. Review the UW’s COVID-19 Vaccinations page for complete details.

A. Check the box that says I am not fully vaccinated, and I am declaring an exemption…
B. In the Vaccination Exemption field, select the reason for your exemption.
C. Check both the I declare… and I acknowledge… boxes.

Continue on to step 5, below.

5. Select the OK button. (Note that there may be a momentary delay at this point.)

6. Select the Done button to submit your attestation.


  • You’re all set!

    If you need to update your vaccine information in the future, simply use the COVID-19 Vaccination Attestation link found on your Workday homepage, then on the COVID-19 Vaccine Verification page, select “Edit” at the bottom of the form to make changes. Don’t forget to select “OK” then “Done” to submit your updates.
